TREKK launched in 2011 as a knowledge mobilization network through the Government of Canada’s Networks of Centres of Excellence Knowledge Mobilization Initiative to address these critical knowledge gaps. Trekk contains bottom line recommendations, ordersets and algorithms for a variety of topics.

 

Do Not Use Abbreviations

The use of some abbreviations, symbols, and dose designations has been identified as an underlying cause of serious, or fatal medication errors. ISMP has a list of abbreviations that should never be used when communicating medication information.
